Responsibilities*
Provide comprehensive nursing case management in a multidisciplinary cardiovascular clinic for the cardiology oncology patient population. Coordinate care and follow-up for patients with complex cardiovascular disorders. Provide instruction and consultation to members of the health care team. Develop partnerships with other clinical programs to standardize patient care processes. Facilitate and maintain quality improvement activities with-in the cardiology-oncology program.

Clinical duties include:
  • Use clinical knowledge and reasoning, and critical thinking to assess and evaluate needs of cardiac-oncology and general cardiology management.
  • Work collaboratively with physicians, advanced practice providers, discharge coordinators, and other consulting services to provide high quality effective patient care.
  • Direct outpatient clinical support through telephone triage and post discharge management, supporting a seamless delivery of care.
  • Participate in the coordination of care, including testing, medication management, collaborative planning of care, support of self-management by patients/families, management of emergency situations, etc.
  • Document care and patient progress through detailed electronic patient records.
  • Assess and provide patient education for patients/families post discharge.
  • Develop metrics to support nursing process to evaluate the effectiveness of the care delivery model eg. decrease readmissions.
  • Identifies the deficits and learning needs of a population and creatively addresses them utilizing evidence based strategies.
  • Collaborates with professional colleagues outside of Michigan Medicine, nationally and internationally.
  • Develops new protocols based on current practice evidence.

Salary & Nursing Framework Level:This UMPNC RN posting is posted as Nursing Framework LEVEL C.Actual Nursing Framework LEVEL and salary will be determined at time of hire.Nursing Framework levels range from Level A to Level F.

  • Evening Shift Differential- $2.35 per hour
  • Night Shift Differential- $3.20 per hour

Required Qualifications*
  • Current license to practice nursing in Michigan.
  • One (1) year of recent (within the last five (5) years) ambulatory care experience. This ambulatory care experience must include telephone triage.
  • Minimum of three (3) years of recent (within the last five (5) years) experience in managing long term cardiac conditions.
  • Clinical leadership experience such as charge nurse, preceptor experience, or clinic instructor.
  • Participation in unit committees or quality projects that demonstrate the ability to work collaborative with and positively contribute to a team approach to patient care.

Desired Qualifications*
  • BSN
  • Two (2) years of recent (within the last five (5) years) ambulatory care experience. This ambulatory care experience must include telephone triage.
  • Greater than five (5) year's experience in managing long term cardiac conditions.
  • One (1) or more years of oncology experience.
  • Previous experience in cardiovascular ICU, cardiac ICU or cardiac step-down.
  • Previous experience in working from and developing evidence-based clinical protocols.
  • Must demonstrate expert communication, organization and prioritization skills.
  • Knowledge of the University of Michigan Hospital Nursing policies and practice.
  • Experience with MiChart and/or EPIC and/or super user for electronic medical record.
  • Previous participation in research
  • Demonstrated ability to work collaboratively with and positively contribute to a eam approach to patient care, willingness to flex and cover colleagues as needed.
  • Demonstrated high standards of personal and professional conduct and attendance as evidenced by interviews, references, and performance evaluations.
  • Experiences with Dobutamine and stress testing
  • Experience with IV Skills

Work Schedule
Hours: 40 hrs/wks
Shift: Variable days and hours
Location: CVC, Northville Health Center, and any additional locations as needed
All new employees will be expected to float to a designated unit(s) in times of low census.
